We are now accepting Applications for the Low-Cost Spay/Neuter Program.
How to Apply:
- You can print out the application with the button below or from the Online Forms section of our website. Then, drop off the forms at the clinic with your proof of eligibility.
- Email Application and proof of eligibility to our office manager’s email.
- Come by the Clinic to fill out the application. Don’t forget your proof of eligibility. See application for eligibility requirements. Each Household that gets approved may receive up to 2 vouchers.
